Job Title: Case Manager
Start Date: Negotiate
Compensation: $42,000 – $46,000

Major responsibilities:

- Implement Gift of Hope policies and procedures,
- Monitor Program documentation for compliance with TFA, Medicaid, and DBHDS licensure requirements;
- Be available, as needed, to carry a company pager to resolve emergencies issues and provide staff and resident support;
- Plan and coordinate residents’ admissions and discharges;
- Schedule, coordinate, and attend residents’ medical, dental, counseling, psychiatric, and all other appointments, as needed;
- Write and implement residents’ assessments, individual service plans, monthly/quarterly reports, and discharge summaries;
- Maintain ongoing communicate with the residents’ legal guardian/parent(s), family, and other community stakeholders, as applicable;
- Plan, coordinate, and attend Resident Treatment Team meetings;
- Support residents by helping them develop positive interpersonal relationships, behaviors, and habits to meet treatment goals;
- Keep Program Director, Program Manager, and other applicable staff informed of resident needs, program issues, and emergencies, as needed;
- Attend Staff meetings and Program required training;
- Provide ongoing staff training to meet TFA, Medicaid, and DBHDS licensing requirements;
- Monitor facility needs to ensure the Program operates within licensing standards;
- Maintain QMHP-C licensure requirements.
Physical requirement:
This position may require tasks such as lifting, running, and kneeling.
Schedule:
As needed, Monday through Friday, 9:00 am – 5:00 pm, some evenings and weekend hours may be required.
Supervision:
This position is supervised by the Program Director, Program Manager & Senior Case Manager.
Qualifications:

- Must meet personal requirements of VCBC and DMV record check;
- Master’s degree in social work, psychology, or counseling;
- Baccalaureate degree from an accredited college or university in psychology, counseling, or social work with documented work experience or baccalaureate degree and three years of professional experience working with children;
- Ability to communicate well with residents, families, staff, supervisors, and other professionals;
- Ability to assimilate information and utilize it to benefit the Program;
- Ability to produce reports, letters, and plans using available office equipment.

Major responsibilities:

- Maintain petty cash ledgers and other assigned financial responsibilities
- Assist in compiling the information needed for billing and payroll
- Plan and coordinate residents’ admissions and discharges;
- Assist in the implementation and monitoring of all residential services plans
- Support residents by helping them develop positive interpersonal relationships, behaviors, and habits to meet treatment goals;
- Attend Staff meetings and Program required training;
- Monitor facility needs to ensure the Program operates within licensing standards;
- Coordinating recreational/community service activities;
- Development of daily living program within each childcare unit;
- Provide monthly staff evaluation and documentation to meet TFA standards;
- Conduct monthly safety checks for residences and vehicles;
- Maintain QMHP-C licensure requirements.

Qualifications:

- A baccalaureate degree in social work or psychology from an accredited college or university and two years of experience working with children, at least one of which shall be working in a children’s residential facility; or
- A high school diploma or GED and a minimum of 5 years of professional experience working with children, with at least two years being in a children’s residential facility, or a combination of education and experience working with children as approved by the lead regulatory authority;

Major responsibilities:

- Provide behavioral support in accordance with the program plans and compliance with both company policy and regulations.
- Provide transportation to appointments and activities.
- Complete the required documentation of program activities.
- Ensure that the residences are well maintained; report any maintenance, health, or safety concerns promptly.
- Monitor health and general well-being of residents, including possible side effects of medication; assist in administering medication in accordance with procedures.
- Help to meet the goals and objectives of any required service plan.
- Development of acceptable interpersonal communication and socialization skills.
Physical requirement:

- This position may require tasks such as lifting, running, kneeling, and occasionally lifting up to 20 pounds.
- Staff is required to perform restraints using “Handle with Care” when needed for behavioral support.
Schedule:
As needed, typically Monday through Sunday, forty (40) hours a week, some overtime as needed.
Supervision:
Supervision is received from the supervisor.
Qualifications:

- Have a baccalaureate degree in human services.
- Have an associate degree and three-month experience working with children, or a high school diploma or GED, and have six months of experience working with children.
- Must demonstrate, through a combination of education and/or experience, the knowledge and ability to provide training in independent living skills, community integration skills and related programs.

Job Title: Mental Health Skill-Building Worker

Major responsibilities:

- Provide behavioral support in accordance with the program plans and in compliance with company policy and regulations.
- Develop and implement Individual Service Plans to meet client-specific needs outlined in the Needs Assessment and monitor the client’s progress through the development and completion of a Quarterly Review of treatment goals.
- Update the ISP as needed to address any additional needs identified and document the completion of service plan goals.
- Complete daily documentation pertaining to MHSS in accordance with program policies.
- Provide MHSS to include: one-on-one training and support in areas related to independent living in the home and community. Skill areas may include: obtaining/maintaining housing, money management, taking medication as prescribed, grocery shopping, nutrition, cooking, hygiene, safety, coping skills, connection to community resources, social skills, and other areas indicated in the Needs Assessment and ISP.
- Use of MHSB staff vehicle to provide transportation to meet client’s service plan goals.
- Meet with MHSB Supervisor on a consistent basis to identify additional client needs, review documentation, and address staff concerns.
- Monitor clients’ general well-being to ensure clients’ personal needs are met and identify additional support areas.
- Report health and safety needs to the supervisor and/or appropriate persons.
- Development of acceptable interpersonal communication and socialization skills
- Maintain qualifications for QMHP-A.

Qualifications:

- Master’s degree in psychology, social work, counseling, substance abuse, or marriage and family therapy and commensurate work experience to meet criteria for QMHP-A or QMHP-T, as set forth by the Department of Health Professionals Board of Counseling.
- Have a master’s or baccalaureate degree in human services and supervised work experience, which meets the criteria for QMHP-A or QMHP-T, as set forth by the Department of Health Professionals Board of Counseling.
- Baccalaureate degree in an unrelated field that includes at least 15-semester credits or 22 quarterly hours in a human service field and with commensurate work experience to meet criteria for a QMHP-A or QMHP-T as set forth by the Department of Health Professionals Board of Counseling.
- Must demonstrate through a combination of education and/or experience, the knowledge and ability to provide support and training in life skills and support
- Ability to communicate well with residents, families, staff, supervisors, and other professionals.
- Ability to assimilate information and utilize it to benefit the program.
